There are a few reasons a Provider might not be eligible:
- The Provider type isn't recognized by the CRA as an Authorized Medical Practitioner
- The Provider isn't listed as an Authorized Medical Practitioner in the province the treatment was provided in
- The Provider isn't registered with the applicable regulatory body in the province they're practicing in
